Currently, when a patient completes a form in Cortico that is uploaded to Accuro, two separate entries are created in the patient's Virtual Chart if the PDF upload setting is enabled for that form:

• A Clinical Note (plain text)

• A Document (PDF)

By default, if the PDF upload setting is not enabled, only a Clinical Note (plain text) is created in the patient's Virtual Chart.

Provide an option that allows clinics to choose which version of the completed form is uploaded to the EMR, for example:

• Clinical Note only (plain text)

OR

• Document only (PDF)
